There has a bug report filed here.
When using Menu Editor, some users report not having an "Add to Fast Dial" option in the Tab context menu.
Upon my testing, it only seems to affect some users.

To access the "Options" dialog, go to Tools>Addons. Click on "Menu Editor". Then, click on the "Options" button.

From there, anything is pretty straight forward.
The "Identification" column is what appears in the menu.
The "Visible" column is whether it is being shown.
To hide an item, either click on the Check mark, or click the large "Hide" button on the left hand side.
To show an item, either click on the "X" in the "Visible" column, or click the large "Show" button on the left hand side.
The area that says "Main Context Menu" is a clickable field, and when you click it, it provides you with all the menus to edit.
The "Visible" checkbox next to that chooses whether or not to show the entire menu (i.e., don't show the "File" menu).

The reset button resets all the menu items to their defaults.
