First of all, THANKS A LOT for that wonderful extension, it makes me feel like in OPERA, the browser from which I came to use FF, because OPERA is worsening more and more.

I am using that subject version from SIMPLE MAIL.

In 'Preferences > General > Management of acknowledgements' I have chosen "Ask before sending a response".

IT IS asking and IT IS sending, but there is NO ADDRESS entered, thus that mail is sitting in my SENT box, without any ADDRESS thus NO ONE gets it!

This is clearly a BUG, and an important one too - for me!

Could you please check this?

This is indeed a bug but only for display, if you look at the source of the message you will see that the recipient is specified, so the acknowledgment reaches the recipient well. Anyway this bug will be fixed in the next version

Concerning that BUG-fix for acknowledgments to come in V2.83.2 I would like to propose the following, deduced from notification mails I receive from EUDORA's emailer (as an example only):

I get the acknowledgment mail with the following SUBJECT line: Subject: Notification for "Re: Fwd: RE: Office Action DUE October 30 Our ref. TI06-015 email2 of 2"

The contents of the mail comprises 1. a text message with some relevant informations about the SUBJECT of the received i.e. original mail, about DATE and TIME of reception as well as about the RECEIVER's NAME and 2. a text attachment with informations about the email-program, message-ID, path etc. (see examples below):

1. a text message:
"The message you sent on Sat, 20 Oct 2012 14:21:05 +0200 regarding "Re: Fwd: RE: Office Action DUE October 30 Our ref. TI06-015 email2 of 2" has been displayed by Walter Matthau"

2. a text attachment:
"Reporting-UA: QUALCOMM Windows Eudora Version 7.1.0.9
Final-Recipient: rfc822;
Original-Message-ID: <1TPY38-1c0ZoO0(at)fwd06.mailprovider.com>
Disposition: manual-action/MDN-sent-manually; displayed"

Whereby that text-message 1 is really IMPORTANT, the text-attachment 2 is only an aside.
