First of all, my compliments to the author of this extension. Second, I think FastDial really needs one visual improvement: centering all dials vertically. Third, is this extension opensourced, and if so, where's the repo located?

Thank you. Dials used to be centered vertically in older versions (2.22b, for example) but somebody removed that, not sure why. There's no repository, so far I'm the only developer plus some people provided some fixes.
Regarding empty boxes - are you using Default Fast Dial theme? Maybe you're using some Stylish styles which are causing this?
P.S. I suggest installing version 4.6 (click "display all versions", it fixes several bugs).

We really need a/more active developers for this project.
It's very useful and there really isn't any other addon that fills the role of Fast Dial, so we're stuck with a buggy and outdated addon that doesn't really work as well as it could.

Regarding Github - I don't know what it is and I have never used it. BTW, there're no people so far willing to help.

Github is basically a site like sourceforge/google code, a place where you can host a repository that contains source code for a particular project. Then you can get community members to commit code to the project. It's very useful for open source projects.

The version there includes a fix for vertical dials centering plus "back" button fix. I'm not going to upload this version to AMO till they review version 4.6 - five days passed and no review so far.
Update: Reverted fix for "back" button, because it works incorrectly. Instead, see https://bugzilla.mozilla.org/show_bug.cgi?id=776167

Confirming the bug, it occurs on modified themes.
