What Firefox Version
Can we agree what background you are trying to load, please chose a background from this site. And then I can try to understand your problem.
For a start create a new Firefox Profile
Load only one extension into that Profile - Fast Dial
Then see if it works regarding the background
Please let me know how you get on

Did you try in a new Profile
So this background: dark fire fox
There is a space in the name of this background
Please after downloading rename file without space before using.
Works for me after name change but not before
Have you got small box checked on the right of the browse for background. If so please uncheck

This small box is for "centre image" or "tile"
If centred often it appears that no background is loaded as the centre is so far away
