For in a sense Fast Dial is derived from that idea.
The first name I seem to remember given to FD was Simple Dial and then that was changed to Fast Dial, maybe to be different from Speed Dial.

I think also that it is worth mentioning the Extend Firefox 2 contest when Fast Dial was given a place.
Although I can find references, cannot find the correct page to give you a link.

I think you should also mention UserLogos as it's one of the factors that I think makes Fast Dial more attractive than other alternatives (It's certainly why I switched to fast dial).
