Go to the Thumbnail subtab.
Make the "margin" set to the same.

Set homepage to chrome://fastdial/content/fastdial.html

UPDATE: if not, then please do the following: enter "about:config" in browser address bar, find "javascript.options.showInConsole" preference, set it to true. Then hit Ctrl+T to open a new tab or restart Firefox and then open Error Console (menu Tools > Error Console) and tell me the errors you see there.
